What is it about?

According to AASLD and EASL guidelines, PHT in cirrhotic patients is considered a relative contraindication for HR due to the likelihood of hepatic decompensation, compared to patients without PHT. In our study we researched the result of HR in cirrhotic patients in high volume specialized center in liver, to see if we can do this procedure in cirrhotic patients with HCC as a curative procedure.

Why is it important?

Hepatic resection (HR) in cirrhotic patients with hepatocellular carcinoma (HCC) and portal hypertension (PHT) is not recommended, according to international guidelines. But HR in Child–Pugh class A cirrhotic patients with PHT is a safe and effective procedure with good short- and long-term outcomes in comparison to patients without PHT, especially those with limited liver resection.

Perspectives

In specialized well equipped center with good patient selection, we can do safe hepatic resection for cirrhotic patients and portal hypertension patients with liver tumor as a curative treatment, with accepted outcome specially after the new antiviral drugs for HCV.
