What is it about?

Chiari malformation type I is typically treated by foramen magnum decompression. But C1-C2 distraction or endoscope can be options for treatment.

Why is it important?

Occiput-cervical fixation is often applied for complex CVJ pathology, including CM1 w/ basilar invagination. But we can avoid occiput involvement for fixation through elegant endoscopic anterior decompression.

Perspectives

CM1 w/ basilar invagination is sometimes treated insufficiently by simple O-C fixation. We had pursued the greatest decompression with the maximum motor unit preservation by endoscope usage.
