What is it about?

We analyzed 249 Staphylococcus aureus isolates to detect low-level oxacillin resistance that may be missed in routine testing. Twelve borderline-resistant strains were identified, including eleven carrying the mecA resistance gene. The dominant types were PVL-positive t437-SCCmecIV and coagulase-negative t037-SCCmecIII. Most originated from skin infections and diabetic foot ulcers and were resistant to multiple antibiotics.

Why is it important?

These findings reveal a potentially overlooked group of MRSA strains that may escape standard diagnostic procedures. Incorrect classification could lead to inappropriate antibiotic treatment and contribute to the continued spread of resistant bacteria. The study highlights the importance of combining routine susceptibility testing with molecular methods, particularly in patients with chronic wounds, skin infections and diabetic foot ulcers.

Perspectives

Improved detection and continued surveillance of borderline oxacillin-resistant S. aureus could support faster diagnosis, more effective treatment decisions and better infection control.
