What is it about?

Because of the unique characteristics of elderly people with T2DM, therapeutic strategy and focus should be tailored to suit this population. We raise the concern of glycemic variability, and discuss the rationale for the selection of current options in managing this patient population.

Why is it important?

The target of glycemic management in elderly patients with T2DM has been focused on preventing frailty and preserving functional independence. Some combination therapies have demonstrated desirable effects, such as A1C lowering, hypoglycemia and glycemic variabliity reducing, in middle-aged adults and thus may be appropriate for older patients.

Perspectives

A safe intensification process and selection of antidiabetic drugs with lower glycemic variability and lower hypogylcemia risk may achieve this goal.
