What is it about?
Disease management programs for chronic diseases (eg diabetes, coronary heart disease) are introduced to improve health of persons, quality of life, reduce risk for disease related complications and costs. In this study, the Austrian disease management program „Therapie aktiv–Diabetes im Griff (http://www.therapie-aktiv.at)“ for patients with type 2 diabetes mellitus was investigated concerning the long-term follow up of the early and the effectiveness of the established program.

Why is it important?
Our findings show that participation in the program can improve survival and reduce health care costs for long-term participants. Better healthcare utilization (including physician contacts and laboratory parameter testing) was observed for participants compared to non-participants especially at the beginning of the program participation.
