What is it about?

The paper is about the pathogenesis of secondary hyperparathyroidism (SHPT) in chronic kidney disease (CKD).

Why is it important?

The paper provides a unifying explanation for two cardinal features of SHPT: (1) a high parathyroid hormone concentration ([PTH]) is required to maintain a normal plasma calcium concentration; (2) [PTH] rises and falls with dietary phosphate intake.

Perspectives

Animal experiments done decades ago showed that [PTH] can be kept normal in CKD if phosphate intake is reduced in proportion to the reduction in glomerular filtration rate. Our paper provides an explanation for that observation.
