Implementation fidelity and local adaptations have established their positions, but do they actually fit together? This study examines if there is any hope of bringing strict implementation strategies into diverse agrarian and pastoral settings, and if core intervention designs and contextual flexibility can complement each other.

Why is it important?
We demonstrate how balancing implementation fidelity and local adaptation optimize primary healthcare in diverse agrarian and pastoral settings. This is important in implementation research, to anticipate real-world service delivery challenges and design health strategies to exhibit targeted outcomes and performance. Two significant findings are that: a) implementation strategies often do not rigidly adhere to their original design without modifications in dynamic contexts and b) we identify and quantify a preference for health programs to maintain core fidelity while adopting flexible, context-specific solutions in real-world field operations
Perspectives
Writing this article has been a valuable learning experience in implementation research and has significantly enhanced my understanding of documenting implementation strategy adaptations. I believe it will also make an important contribution to the growing body of evidence on documenting implementation strategy adaptations in low- and middle-income countries.
