What is it about?
Older adults who are physically inactive face an increased risk of cardiovascular problems, highlighting the need for safe, non-drug interventions. We investigated whether a single session of intermittent hypoxia-hyperoxia - alternating periods of breathing air with lower and higher oxygen levels - could produce immediate cardiovascular changes. Sixteen sedentary older adults were randomly assigned to receive either the oxygen intervention or a sham session with normal air. After the session, participants receiving the intervention showed changes in beat-to-beat heart rhythm consistent with more favourable autonomic regulation, together with lower heart rate and lower systolic and diastolic blood pressure. Oxygen saturation also increased slightly, and no adverse events were reported. Because this was a small pilot study and measurements were taken only immediately after the intervention, larger studies are required to determine whether these effects are lasting and clinically meaningful.

Why is it important?
This pilot randomized trial provides preliminary evidence that a brief, non-invasive and drug-free intervention may acutely influence cardiovascular autonomic regulation and blood pressure in sedentary older adults. This is relevant because impaired autonomic control and high blood pressure are important cardiovascular risk factors during ageing. The findings identify a promising direction for future research but should not yet be interpreted as evidence supporting routine clinical use. Larger trials involving repeated sessions and longer follow-up periods are needed to confirm the effects, establish their clinical relevance and identify which older adults are most likely to benefit.
Perspectives
As a physiotherapist and researcher, I am particularly interested in safe, non-pharmacological strategies that could support healthy ageing. This study allowed us to explore a technique largely associated with sport and performance within a residential care setting. Observing clear physiological changes after a single monitored session was encouraging, although the small sample and the absence of longer-term follow-up mean that the results must be interpreted cautiously. I hope this pilot study serves as a starting point for larger trials examining repeated exposure, longer-term cardiovascular outcomes and the characteristics of the people most likely to benefit.
