Preoperative diagnosis of the severity of acute appendicitis is difficult even with current high-quality imagings. The article analyzes its difficulty and shows which scoring system seems useful. The surgeon should combine several factors for evaluation and not rely too much on imaging.

Several scoring systems have been reported but I think simple and easy to use are very important. It seems impossible to make a perfect scoring system now, so we should utilize the current reported one, not publish a new one.
