What is it about?
Off label prescribing is the use of a medicinal product outside the conditions of its licence. The licence has some value in guiding safe prescribing. This article presents a case for off-label prescribing when it is based on the best evidence. It also examines the definition of evidence based practice in off-label prescribing.

Why is it important?
There is little evidence regarding off-label prescribing by nurse prescribers. Some evidence suggests a deference to medical colleagues in off-label prescribing decisions. Nurse prescribers can take steps to address their competence to prescribe off-label.
Perspectives
I hope this article encourages nurse prescribers to develop their understanding of true evidence based prescribing. It has challenged me to reflect on my own practice of prescribing off-label where this will achieve best outcomes for patients.
