What is it about?
The distally-based or reverse flow anterolateral thigh flap which supplied by the descending branch of the lateral circumflex femoral artery and its anastomotic is a viable option in patients who present with soft tissue defects around the knee and upper third of the leg.

Why is it important?
The distally-based anterolateral thigh flap resulted in constant and reliable references about perforator sites and suitable for knee reconstruction.
Perspectives
This type of flap is not frequently used, as many studies have revealed that it can lead to complications including partial flap necrosis, venous congestion, and the necessity for secondary or revised procedures. But for my opinion, this is very easy to harvest and are more advantages in terms of donor site morbidity
