Why is it important?

In selected patients at high risk of SSI and/or wound breakdown, use of incisional NPWT has been shown to actively manage clean, closed surgical incisions.

Perspectives

When looking into what is really known bout seroma prevention and compressive Dressings in surgery Little can be found that scientifically validates any of the current postoperative external prevention measures so far. Most of the knowledge is anecdotical and based on personal opinions rahter than on scientific dara. When experimenting with topically applied negative pressure on the skin over closed wounds, such as in abdominoplasty, we noticed a significant reduction in seroma formation. At the same time patients reported a positive Feeling of stabilizing their abdomina wall. We then measured perfusion parameteers under such an occlusive negative Dressing over closed incisions. We could demonstrate an enhancement in Skin Perfusion, Oxygen satruation and reduciton in postcapillary hemoglobin Content in the treated area. Therefore we conclude that external NPWT wound dressings over the ventral and lateral trunk following post-bariatric abdominal dermolipectomy, as shown in our results lead to a significant reduction in exudate formation, earlier drain removal, and decreased length of hospitalisation, when compared with conventional treatment.
