What is it about?

District Nurses in Wales have developed the Welsh Levels of Care Tool - an acuity and dependency tool which can be used by district nurses to give patients on an active DN caseload a acuity and dependency score, providing a way of capturing the patient’s level of complexity, dependency, unpredictability and risk which would help inform decisions on nurse staffing levels and caseload management.

Why is it important?

District Nursing services often face challenges in articulating the complexity of their caseloads. The WLOC Acuity and Dependency Tool can help district nurses to describe and evidence the acuity and dependency of patients on their caseload, using the tool to support their professional judgement and to consistently measure patient acuity.

Perspectives

This work would not have been possible without the interaction and engagement of district nurses across Wales, and demonstrates how working collaboratively can enable standardised initiatives to be developed.
