Why is it important?

A collaborative approach has led to a reduction in acute hospital admissions because GPs and specialist teams can pool their wealth of knowledge and complement each other to provide the best of person-centred care.

Perspectives

Interviewing the doctor was a great experience for me to understand the model better and I was pleased to be able to write this and share the impactful collaborative approach with a wider audience.
