The incidental discovery of pre-clinical CT abnormalities has led to the recent designation of Interstitial lung abnormalities (ILAs), a purely radiological entity, in which monitoring of those at risk of progression is recommended. In this review we explore links between ILAs and interstitial lung diseases (ILDs) and we underline the importance of an early and accurate ILD diagnosis if there is evidence of clinical significance, regardless of whether or not CT abnormalities are identified fortuitously.
