The published experience of lung transplantation in acute respiratory distress syndrome, a sudden and severe disease leading to respiratory failure is limited to historical case series according. This series summarizes the shared experience about this rare indication for lung transplantation across Europe.

Why is it important?
During the last decade 40 patients in 48 European centres were selected for lung transplantation. Thirty one of 40 patients survived until transplantation. One year survival was 71 % after lung transplantation. Outcome has considerably improved in the most recent era 2015 -2019.
Perspectives
Lung transplantation as a treatment for Acute Respiratory Distress Syndrome is possible but should be carefully performed given the donor shortage in most countries. Disdavantaging other patients on waitlists must be avoided.
