What is it about?

JLB is a long-cannula venous catheter we designed for cannulation of medium and large bore veins. IJV, Subclavian, Basilic, Cephalic, Brachial veins are all suitable. The insertion technique is intended as over-the-needle and ultrasound-guided. Since the device war released among many hospitals in northern Italy (June 2015) we retrospectively collected data from 1000 insertions, in order to investigate the safety of the device, the technical learning curve, the demographics of the patients. JLB is available in three different lengths (6 - 7 - 8 cm) and four different gauges (14 - 16 - 17 - 18); in many patients with difficult intravenous access (DIVA), JLB could be a quick and valuable alternative to more invasive devices (such as CVCs), since it does not require neither Seldinger technique nor ICU environment.

Why is it important?

Even in the emergency setting or in the fragile patient, central venous accesses could be considered an over-treatment, expensive, invasive and time-consuming. Catheterization of an IJV using our JLB offers the advantages of a regular single-lumen CVCs, except for CVP monitoring; furthermore, the insertion technique is significatively quicker and easier-to-learn. Our preliminary data suggests that JLB could be the "missing ring" between regular peripheral venous accesses (PIVs) and invasive venous lines (such as CVCs).

Perspectives

A 2.0 version of the JLB (provided with a safe-box insert and a blood-stop valve) is currently under development.
