What is it about?
Families caring for children with rare, life-limiting conditions often face uncertainty, complex decisions and significant emotional, social and practical challenges. This commentary discusses research exploring a structured paediatric advance care planning intervention that provided families with dedicated time and support to discuss their hopes, concerns and future care preferences. The study showed that these conversations helped families feel more supported and empowered, while identifying important priorities such as having personal time, understanding what the future might hold and managing financial or work-related pressures.

Why is it important?
Advance care planning is often associated with clinical decisions and end-of-life care, but this research highlights that families' priorities are frequently much broader. Parents and caregivers need opportunities to talk openly about uncertainty, emotional wellbeing, family responsibilities and the practical realities of caring for a child with a life-limiting condition. Providing structured advance care planning can help families make informed decisions, improve communication and ensure that care reflects what matters most to them.
Perspectives
Effective advance care planning is about more than documenting treatment preferences. It creates space for meaningful conversations that acknowledge the emotional, social, practical and spiritual dimensions of family life. Healthcare professionals supporting these discussions require skills that extend beyond clinical expertise, enabling them to build trust, explore uncertainty and empower families to make choices aligned with their values and goals. By taking a holistic, family-centred approach, advance care planning can improve experiences for children with life-limiting conditions and the people who care for them.
