What is it about?
We conducted a scoping review to explore the extent of patient engagement in the development of best practice reports related to the transition from hospital to home. The transition from hospital to home is an important, yet vulnerable exchange point that if not properly handled can lead to adverse clinical events and preventable hospital readmissions. Often patients are not involved in their care transitions planning, which can result in unmet needs once home. Including the patient voice in guidance development may address some of these known gaps in care.
