What is it about?

Communicating with families whose baby may die before or shortly after birth is one of the most challenging aspects of perinatal care. This commentary discusses research exploring the use of scenario-based simulation to help nurses develop confidence and competence in palliative care communication. The study found that while both traditional teaching and simulation improved learning, simulation-based education provided greater benefits by allowing nurses to practise realistic conversations, receive feedback and reflect on their communication skills in a safe learning environment.

Why is it important?

Many nurses report feeling underprepared to have conversations about poor prognoses, end-of-life care and difficult decision-making with families. These discussions require not only knowledge but also empathy, compassion and confidence. The study highlights how simulation-based education can help nurses develop these essential skills before they encounter real-life situations. Better communication can improve family experiences, support informed decision-making and help parents feel heard, respected and supported during exceptionally difficult circumstances.

Perspectives

Perinatal palliative care requires healthcare professionals to combine clinical expertise with sensitive, compassionate communication. Simulation-based education offers a valuable opportunity for nurses to practise difficult conversations, explore emotional responses and develop confidence in supporting families facing uncertainty, loss and grief. The commentary argues that simulation should complement traditional teaching approaches, creating learning experiences that are authentic, reflective and emotionally meaningful. As demand for perinatal palliative care grows, investment in high-quality communication training will be essential to ensure that families receive care that is not only clinically excellent but also compassionate and family-centred.
