What is it about?

The standard treatment for prostate cancer patients with lymph nodal recurrence is hormonal therapy until resistance, and chemotherapy as second-line therapy achieving only a progression delay. We have shown in this paper that 11 C-Choline-PET/CT-guided salvage tomotherapy obtain a good local control, a high rate of biochemical response with good clinical relapse free survival, in prostate cancer lymph nodal relapses after radical and salvage treatments. Acute toxicity is low and late toxicity is acceptable.

Why is it important?

Lymph nodal recurrence in prostate cancer patients is considered a metastatic disease. The addition of loco-regional therapy and the reduction of the burden of disease will probably enhance the effectiveness of systemic therapies. Furthermore, radiotherapy has a potential role as definitive therapy in those patients presenting confined disease, reducing the need for systemic therapies, which are subject to both side effects and costs.

Perspectives

PSMA PET/CT could help to a better selection of oligometastatic patients, improving the results of salvage therapies.
