What is it about?

Service integration, careworker autonomy, frequent reassessment, and continuity of care are important elements of effective social care for people with dementia at home.   Who needs to know Policy-makers, commissioners, and providers of social care for people with dementia at home   What we have found Research studies support the finding that important elements of effective social care for people with dementia at home include: * Integration of social and health services * Careworkers who can make autonomous decisions * Careworkers who can provide continuity of input to the person with dementia * Careworkers who can reassess the person with dementia frequently   Findings and implications  Our literature review found evidence in support of the finding that important elements of effective social care for people with dementia at home include: * Integration of social and health services * Careworkers who can make autonomous decisions * Careworkers who can provide continuity of input to the person with dementia * Careworkers who can reassess the person with dementia frequently   This evidence base challenges policy-makers, commissioners and providers of such services increasingly to ensure that services are in line with this evidence base and, hence, most effectively support people with dementia at home.   Whilst we did not find robust economic studies in this area, we did find evidence that the increased costs that might be required to deliver such services over inferior ones could be offset by fewer admissions to hospital and delayed moves into institutional care.
