What is it about?

This article is about one of the models designed to cope with the rising economic burden of chronic disease. The increasing number of people with complex care needs requires the development of delivery systems that bring together teams of professionals and skills from both the health and social care sectors. The Trafford CCG and CSC have embarked on a challenging journey to build such a system.

Why is it important?

Physicians, hospitals, and health systems are under increasing financial, market, and regulatory pressure to improve management of chronic care diseases, eliminate unnecessary costs, adhere to evidence-based medical guidelines, and deliver high-quality, patient-centered care. Searching for the solutions of these issues takes a central place in health and social care delivery all over the world. This article provides a model for such a solution.

Perspectives

This is my point of view on a recent development of an innovative coordinated care solution named the Care Coordination Centre (CCC), which targets coordinated care arrangements for populations expected to benefit the most. This is my assessment of the economic benefits of a coordinated care model targeting less costly interventions, reduction in readmissions, reduced unplanned admissions, increased time allocation efficiency of healthcare staff, and the overall reduction in per-patient costs. As the health and social care systems in the US, the UK, and globally, are facing the biggest challenge yet in their history - payers, providers, governments, and communities need to work together to build better coordination and integration mechanisms to manage the increasing demand on health and social care in a period of stagnant health and social care budgets. The answer is - designing new innovative models of coordinated care at the local economy level, as the health service systems are undergoing transformation from a supply to a demand-based healthcare model.
