What is it about?
It is suggested that lactate-buffered replacement fluids during haemofiltration can worsen metabolic acidosis in critically ill patients. We studied this in the critically ill and found that acidosis did not worsen using lactate-buffered replacement fluid in these patients

Why is it important?
It is reassuring that metabolic acidosis is not made worse by using lactate-buffered replacement fluids during haemofiltration in the critically ill
Perspectives
There are many misconceptions about what replacement fluid to use during haemofiltration. Lactate based fluids are perceived harmful for critically ill. This paper should reassure clinicians in that regard.
