What is it about?

In this case we reported a cerebral abscess by Streptococcus Intermedious with atypical features on conventional images: corpus callosum involvement, dark and discontinuous margins on T2 images, irregular enhancement, great size and mass effect. All of this characteristics are also in high grade glioma. But the study with advanced magnetic techniques resulted useful, mainly with spectroscopy.

Why is it important?

In this patient, the diagnosis was not simple for the age and because for the absence of identifiable risk factors for Streptococcus intermedius infections: no fever, no immunocompromised status, no lung, dental infections neither ear infections. Colonoscopy with polypectomy was undertaken in the previous 6 months.

Perspectives

MRI spectroscopy is an important diagnostic tool for abscess diagnosis, also when other MRI sequences are ambiguous. We hope that the advancement on MRI can definitively remove the use of biopsy.
