What is it about?

This paper describes the clinical outcomes, lessons learned, facilitators, and barriers of developing a holistic clinic to support the medical, behavioral, and resource needs of youth with diabetes and their families.

Why is it important?

Including behavioral health services with medical care in a pediatric diabetes clinic is rarely achieved. This program evaluation shows that using the Iowa Model for implementation supported the planning, delivery, and evaluation of this newly developed clinic, promoting team cohesion, consensus, ongoing administrative support, and positive patient and family outcomes.

Perspectives

In health care, creating a team is often seen as setting up a meeting. Team building is an active, engaged process to support relationships, workflow, and respect for others' roles and practices.
