We do repeated impression cytology to assay the level of transcripts in the conjunctiva at different time points after glaucoma surgery.

Why is it important?

Inflammatory gene expression is significantly downregulated years after trabeculectomy (in six critical genes). This shows that glaucoma surgery may be preferable to long term chronic glaucoma medications that induce inflammation.

Perspectives

The timing of glaucoma surgery is an important issue in patients requiring multiple or chronic glaucoma eyedrops. These eyedrops need to be made safer, and for patients with poor ocular surface, earlier surgery may be warranted. There should be interdisciplinary joint management of these patients between glaucoma and ocular surface departments.
