Impact of New York State’s Health Home Model on Health Care Utilization

• • The Affordable Care Act established Medicaid health homes to provide care management and care coordination for high-cost, high-need individuals, including many with serious mental illness. • Enrollment in a New York State health home model was associated with reduced substance abuse and mental health inpatient utilization and increased engagement in mental health outpatient care and behavioral and non-behavioral pharmacy utilization. • Further research is needed to determine the impacts of the health home model on patient outcomes.
