What is it about?

Electrosurgery uses high-frequency currents. For a given total amount of energy delivered, depth of burn and haemostasis are in parallel with low power and long time of application. Therefore, power must be choosen in keeping with the task: high for small polyps destruction, low for thin pedicles or for destroying cancer, very low for coagulating selectively capillaries in radic proctitis.

Why is it important?

Open surgery teaches quickly surgeons. High power carbonizes superficially without haemostasis, according to grasped tissue's thickness. However, endoscopists fail to percieve the in depth effect of coagulation and have largly forgotten the physical laws of high frequency current. So that the false concept of low intensity being securefor polypectomy is very popular. It is responsible for low efficiency of hot forceps if application time fails to be long enough and for perforation, should the opposite occur. Technical details for efficient and safe use of hot forceps, snaring, cancer coagulation and treatment of radic proctitis are specified, based on a 35 y. experience with approximatly 30,000 polypectomy without any significant complication, defined as need for surgery.

Perspectives

Backward movements are not rare in medicine. They are frequently linked to hyperspecialization and a false concept of common sens. These methods could allow a large improvement in efficiency and safety but people are especially attached to a method as its use is more ancient. Should logic improve practices!
