What is it about?

The increase in disability can be slowed down by adjunctive therapy with daily oral doses of high-dose vitamin D. At the same time, the occurence of comorbidities must be prevented. The well-know immunological mechanisms of 1,25(OH)2D3, the active form of vitamin D, are under discussion as an essential factor in the defense against infectctions, especially SARS-CoV-2. Vitamin D and its metabolites inactivate viruses, increase the antimicrobial peptide cathelicidin, defensins, reduce the risk of a cytokine storm, and descrease pro-inflammatory cytokines.

Why is it important?

Because vitamin D has a protective effect on multiple sclerosis through numerous mechanisms and can prevent an infection with COVID-19 or mitigate its acute course, all multiple sclerosis patients should be offered this add-on-therapy. Only a few guidelines recommend this add-on- therapy, although the patho-immunological mechanisms have bee know for decades.

Perspectives

SARS-CoV-2 infection will continue to play a role in care. The multifactorial nature of vitamin D could alleviate some of people`s suffering.Therapists should not miss this opportunity.
