What is it about?

Examining 330 primary care practices and 185,000 patients over three years, the authors identified 18 care management processes that significantly improved diabetes outcomes. Three of the 18 processes provided half of the total improvement. Primary care practices can improve diabetes outcomes by reminding patients due for testing, providing clinician reminders during visits, and having a non-clinician provide follow-up after visits.
