What is it about?

This study compared two ways primary care providers (PCP) worked with patients with diabetes to help them manage the disease. One way PCPs worked with patients with diabetes was to provide regular office visits to check-in and manage any health care issues that arose (usual care). The second way added telehealth or in-person visits with a pharmacist who provided additional disease management coaching and made medication recommendations to the PCP as needed. PCPs used their own discretion to decide which patients to refer to pharmacist coaching. We found that patients who participated in the combined PCP visits plus pharmacist coaching had improved diabetes control.
