Older People With Type 2 Diabetes–Individualising Management With a Specialised Community Team (OPTIMISE): Perspectives of Participants on Care

What is it about?

Older people with type 2 diabetes participated in a person-centred, technology enabled program linking hospital based endocrinologist with credentialed diabetes educator in the individuals home. We found that human connection and relationships were key to positive perspectives of participants entrolled in the study, with the use of technology enhancing care of older people in their homes.
