What is it about?
Diabetes Mellitus type 2 is a complex disease that requires adequate glycemic control from the onset to reduce the incidence of microvascular complications, such as nephropathy, neuropathy, and retinopathy (kidney, nerve, and eye complications, respectively). In a population with multiple health care challenges, implementing a patient-centered diabetes clinic proved to be beneficial for increasing screening rates for diabetes complications and diabetes control.
