In this national study, we found patients who had chosen conservative non-dialytic care when in CKD stage 4 or 5 (pre-dialysis), rarely changed their minds to start dialysis therapy as their disease progressed. This finding challenges a common belief that preferences for treatment of ESKD change markedly. We also found that of the patients who died over a 3 year follow-up period, those on dialysis were less likely to receive palliative care at end of life, and more likely to die in hospital, than those treated conservatively.
