This article gives a logical step by step approach for planning discharge of patients admitted with diabetes. This is targeted for healthcare providers working in hospitals as well as those working in the community who deals with patients after their discharge. This article may also be of use to patients with diabetes and their caregivers

Why is it important?

More than a quarter of patients admitted to hospital have diabetes. Improper blood sugar control after discharge is one of the most important causes of readmission. However there is scarcity of detailed step by step approach for discharge planning for sugar control for patients admitted with diabetes. This article gives an evidence based approach for all healthcare providers when they plan for discharge of patients with diabetes. The aim therefore is not only to improve blood sugar control but also to prevent hospital readmission becuase of poor sugar control thereby reducing hardships of patients and their caregivers

We had a long cherished desire to write an article like this with the aim that it will significantly impact thousands of people with diabetes and their caregivers. Although primarily written for healthcare providers, patients with diabetes and their caregivers may also benefit from this
