What is it about?
Gestational diabetes can cause metabolic disorders and can influence the immune system. In addition, it can affect the infant through the mechanism of metabolic memory, increasing the risk of diabetes later in life. The aim of this systematic review was to describe the immunological characteristics of saliva and maternal milk of women with or without gestational diabetes.

Why is it important?
the immunological characteristics of colostrum and saliva differ between women with GDM and normoglycemic pregnant women. Hyperglycemia during pregnancy influences the production of inflammatory cytokines in saliva and colostrum, suppressing the action of insulin receptors and signaling molecules, while attenuating the expression of IgA. The latter is an important immune protector especially against gastrointestinal and respiratory infections in the neonate.
Perspectives
Our results showed that it is essential to include the immunological evaluation of maternal milk, mainly in those women that developed gestational diabetes and in milk banks. In addition, women with GDM require immediate screening and appropriate and early prenatal management to minimize the effects of the altered production of immunological markers in colostrum on the newborn infant.
