What is it about?

In this original article, we reported the exciting long-term (up to three years follow-up) results of POEM for all types of esophageal achalasia treated in Endoscopy Department of a Greek POEM center. Selective circular myotomy 10-23cm in length was successfully completed without restriction and without severe acute or late complications. Esophagitis was not clinically significant in our POEM series.

Why is it important?

In this study POEM, in contrast to all previous treatments, was easily, safely and successfully applied to all types of esophageal achalasia without exceptions, even to advanced, sigmoid type, achalasia with megaesophagus, failure of all other previous therapies, such as surgical or LHM myotomy or ballon dilatation or previous POEM and also to debilitating elderly patients with serious comorbidities and high risk of surgery. In all these cases POEM provide a long-term solution and was practically the only realistic therapeutic option available.

Perspectives

Our excellent results are in accordance to international data and proved the safety and efficacy of POEM as radical long-term treatment of all types of achalasia. Further experience and long-term follow-up is necessary and awaited.
