Hand-held device with the airflow measure by ratio FEV1/FEV6 are easy to use in outpatient setting to check patients air flow status with added questionnaire in detecting early stage of air way obstruction especially in smoking population.

Our findings suggest that a quite number of under diagnose smoker with air flow limitation but never diagnosed before or being advice to quit smoking by health care professional.
