What is it about?
If unusually large amount of insulin is needed to achieve optimum glycemic control, then the patient is suspected to have insulin resistance. This report describes a rare case who has extreme level of insulin resistance and received 1110 units of insulin per day subcutaneously to control the glucose level.

Why is it important?
We described a case of extreme insulin resistance, which is very rare and only a few reported cases found in the world. This is important for clinical suspicion as the described case was not morbidly obese. We also pointed out the lack of facility and resources to conduct clinical research that hinder the exploration into further clinical knowledge.
