What is it about?

This paper examines if there is synergistic effect of intramuscular 17P in combination with prophylactic cerclage compared to cerclage alone for women with 3 of more prior second trimester losses. We did not find improved perinatal outcomes.

Why is it important?

Our findings are important because women with cervical insufficiency and 3 or more prior second trimester losses have shown to benefit from cerclage placement. In daily clinical practice, these women undergo prophylactic cerclage placement and are also offered adjuvant 17P. Our findings do not support the addition to 17P for these cases.

Perspectives

The authors hope that this article fosters prospective trials assessing the benefit of progesterone (intramuscular or vaginal) or other interventions as pessary to women with cervical insufficiency who already underwent prophylactic cerclage placement due to prior second trimester losses.
