What is it about?

Zoledronate is a potent bisphosphonate for use in patients with osteoporosis but there are concerns on it's long term use in pediatric osteoporosis. In adults, it's use has been found to be related to osteonecrosis of the jaw.The article describes the use of zoledronate in children with osteogenesis imperfecta at a tertiary care centre, especially the safety profile.

Why is it important?

In the report, 26 children with early onset fractures recieved cyclical zoledronate as IV infusions. The side effects and the long term implications are described in the manuscript. Use with regular monitoring can be really beneficial in these patients.

Perspectives

Since zoledronate or bisphosphonate use in children can reduce risk of fractures, improve mobility and hence growth of the osteogenesis imperfecta patients, a trial should be given in these patients. Earlier use may result in better results. Zoledronate has advantage of infrequent administration, and hence convenient for the patients. Oral bisphosphonates can have compliance problems.

As per the index study and various other studies, Zolendronate in safer in chilldren and is effective in reducing the fracture in osteogenesis perfecta. So it can be used as treatment of choice in osteogenesis imperfecta in children.
