What is it about?
With the introduction of the Sysmex XN Haematology analyser the white blood cell differential channels (WNR and WDF) and abnormal cell detection channel (WPC) have been introduced for flagging blasts and abnormal lymphocytes. In this study the performance characteristics of the WBC suspect flags were compared with immunophenotyping results.

Why is it important?
After immunophenotyping subjects were classified as cases with B-ALL, B-CLL, B-NHL, B-NHL/Hairy Cell Variant, MBL and AML according to the WHO criteria for immunophenotyping (2008). Our study resulted in 30 true-positive cases, 6 truenegative cases, 2 false-positive cases and 17 false-negative cases. A MBL was demonstrated in 12 out of the 17 false-negative cases.
Perspectives
To conclude, with XN software version 00.15, the performance of the WPC reflex flag for the detection of small monoclonal B-cell populations is inappropriate. In order to improve the sensitivity of the WPC flagging for the detection of small monoclonal B-cell populations, we recommend to include the absolute lymphocyte count within the XN flagging strategy.
