Improving transitional patient safety: research protocol of the Transitional Incident Prevention Programme

What is it about?

Inaccurate, delayed or absent communication between primary care physicians and hospital physicians severely impedes patient safety. In this paper we evaluate an intervention to enhance patient safety in the transition between primary and hospital care. The intervention includes: a tool to analyse incidents, a committee with primary care physicians and hospital physicians to analyse the incidents and a tool to enhance patients participations. This paper describes the design of the study.
