What is it about?

Among 247 patients who presented with symptoms of TB and screened negative on Xpert, 15.8% had bacteriological confirmed TB. The odds of confirmed TB was twice to five times as high in patients with CXR findings suggestive of TB as compared to those patients with CXR findings not suggestive of TB.

Why is it important?

Defining the performance of CXR (read by clinician or radiologist) to the clinical prediction of active TB among hospitalized patients with symptoms of TB who are Xpert negative is important to define the role of CXR in this population.

Perspectives

In hospitalized people with symptoms of TB and a negative Xpert assay, knowledge of CXR findings (as classified by clinician or radiologist) does not improve the ability of clinicians to predict the presence of bacteriologically confirmed TB beyond what is possible based solely on clinical characteristics. Greater urgency should be paid to implementation of the switch from Xpert to Xpert Ultra in order to use the most sensitive tests in all patients.
