What is it about?

COPD is a frequent With chronic conditions the role of patients changes and patients involvement in their care is a priority. We implemented the living well with COPD self-management programme and could show that helping patients to gain the knowledge, skills, and confidence to be informed, active partners in their care is an effective way to reduce the burden of COPD.

Why is it important?

We raise awareness and improve knowledge about effective self-management and health coaching for COPD This is important because effective patient coaching is not an integral part of COPD management by now and the term self-management is used rather loosely, as another word for “giving information”. .

Perspectives

For myself it was a great experience to see that the implementation was feasible, a pleasure to instill the team approach and moving how patients interacted and shared their experiences and emotions. Last but definitely not least it is worth the investment when you realize it works and patients feel more confident, adopt healthy lifestyle behaviors, and feel empowered to living better with COPD! I hope that our results might motivate transformation and pave the way for better support and training of primary care COPD teams that aim to deliver high quality COPD care and effective chronic disease management.
