Perineural dexmedetomidine with local anaesthetic in stellate ganglion block for post operative pain relief.

Why is it important?

Stellate ganglion has autonomic functions without any sensory and motor functions in the territory supplied., Hence its block relived sympathetic contribution to pain while preserving upper limb sensory and motor functions which are vital for surgeon for assessing nerve injury in the immediate postoperative period. Dexmedetomidine is known for prolonging local anaesthetic block duration and quality thus extending the benefit of stellar ganglion block.

Perspectives

Being in chronic pain for many years it was easier for me to introduce the block in acute pain. However the block is technically challenging for most anaesthetists who may not be not trained for it and may hesitate to perform it. But after some practice it will be easy to perform this block in expert hands. The block may be given near the end of surgery to enhance its impact in post operative pain relief.
