This technique makes totally percutaneous aortic arch endovascular repair using physician-modified standard thoracic endografts eminently feasible and opens up the field like never before. Arch de-branching surgery and its associated morbidity is avoided. Dependence on custom-made branched or fenestrated endografts is also avoided, resulting in considerable cost and time savings. Reduced manipulation and rapid, complete endograft deployment could potentially translate to less cerebral embolization and resultant stroke.
