What is it about?

The difficult decisions that all of us encountered when faced with end-of-life, or chronic illness management whether patient, family member or health professionals involve the contrast of what CAN be done technologically; what SHOULD be done ethically; and what SHOULD be done legally. This conflict has been dubbed as "The Troubling Trichotomy". While some progress has been made during the past 10 years dealing with the Troubling Trichotomy, many of the issues from a decade ago remain unresolved or partially resolved.

Why is it important?

Regardless of our position there is a great possibility that we will all be affected by the Troubling Trichotomy. Background of new technology, particularly related to artificial/specialized Nutrition and Hydration, ethics and law is provided in the article to establish a "level playing field" for all readers whether or not they are in the healthcare arena. Specific cases are reviewed and possible interventions adaptable to the individual situations are provided

Perspectives

For over 5 decades I have been faced with the Troubling Trichotomy on a personal and professional basis. I welcome comments anecdotes and suggestions from others regarding the topics discussed and the issues raised
