What is it about?

When children with Type 1 diabetes enter into the teenage years the topic of transition to an adult diabetes provider begins. It can be a difficult process for some, as well as their families, after developing a long term relationship with a pediatric provider. This article is about a specialized program at Winthrop University Hospital which is designed to help emerging adults and their families make this transition. It highlights a collaborative practice between the pediatric and adult diabetes teams.

Why is it important?

When it comes time for emerging adults to leave their pediatric provider, many are lost to follow up and never make it to an adult provider. This unique program is focused on making the transition process seamless and comfortable for the emerging adult and their family. The pediatric and adult team become partners in transitioning the emerging adult together. This allows for sharing of health history with all parties present.

Perspectives

The Winthrop University Hospital Transition Program for emerging adults with Type 1 diabetes has been a wonderful success. All members of the diabetes team (pediatric and adult) have joined together to ensure that this potentially difficult time is as easy and comfortable as possible for the emerging adult and their family.
